Intent to Withdraw or take a Leave of Absence

We understand that plans can change from time to time. If you need to withdraw from a program or take a Leave of Absence, this form sends a notice to Northwest alerting the Registrar of your intentions. Thank you for taking the time to inform us of this change.

NOTE: In order to be considered eligible for tuition and fee refunds as scheduled on the school calendar, all program withdrawals must be processed through the Registrar’s Office via this official withdrawal form. No refunds will be granted without following this withdrawal process.

Please select the program from which you are intending to withdraw.
Please select the Northwest Partner Network you are connected with.
Upon permanent withdrawal from studies, Northwest will close the student’s online learning portfolio. It is the responsibility of the student to back up and archive any assignments, mentor comments, or other material that the student may wish to keep.
If you are withdrawing permanently please retain access to your Northwest email account for at least one year. Official documents, such as tax slips, will be sent to this email address.
